SERVICE PROVIDED: 1x 60 MINUTE SEMI-PRIVATE SESSION - Weekly 1x 45 MINUTE 1:1 PT SESSION - Weekly COST AND PAYMENT METHOD: DIRECT DEBIT - WEEKLY $119 (excluding transaction fee) FIRST PAYMENT DATE: 7/10/24 . * Your weekly direct debit will be debited on the same day as your first payment date in future. *Minimum Payment Period - 16 Weeks* Coaching Agreement – IGNITE with Kacey -IF YOU WISH TO CHANGE THE FREQUENCY OF YOUR DIRECT, PLEASE CONTACT YOUR COACH TO ARRANGE BEFORE SIGNING THIS AGREEMENT AND SENDING BACK - All services will be paid by Direct Debit and will incur a transaction fee of 2.25% + 30c. if for any reason you need to delay your payment IGNITE with Kacey requires a minimum of 24 hours’ notice so that the change can be implemented. If a payment declines the Direct Debit will be re-direct debited 3 days later and a failed payment fee may apply. TERMS OF AGREEMENT – FOR: FACE TO FACE (INCLUDING SEMI PRIVATE AND 1:1 SESSIONS) + ONLINE / APP The minimum term of this agreement is 16 weeks of full payments or as discussed with your coach, after this term is complete, your payment and program will proceed on a week-to-week basis on the set weekly cost stated in this agreement unless a new agreement is met. • No Refunds, no cooling off period. • Total amount must be paid by the end of the 16 weeks.
